Getting Started

Scheduling an initial consultation takes place in stages –

• First, we want to make sure that we have answered all of your questions about the clinic.
• Second, we ask that you complete a few forms that tell us that you understand our clinic policies, and that you guarantee payment for your visit.
• Third, we will schedule your initial appointment and email you a secure online questionnaire for you to complete prior to your appointment.

This is also where you will find the forms to be completed and submitted prior to scheduling your first appointment.

	Phone Call the main number (415) 551-0520 and leave a message for Dr. Peter Forster. Please speak clearly as the message will be transcribed and emailed to Dr. Forster as well. He is generally available to respond to messages in the morning (around 8am) or in the evening after his last patient (around 5 pm).
